The police force is in the process of investigating whether the Prime Minister and others in Downing Street broke lockdown laws.
A No10 spokesperson said: “We can confirm the Prime Minister has received a questionnaire from the Metropolitan Police.
“He will respond as required.”
The questionnaire comes after news reports claimed the Police had received photographs of Mr Johnson, 57, attending parties during the lockdown.
One image allegedly shows Mr Johnson holding a can of beer.
Earlier this week, Scotland Yard said a team of its officers will start to contact more than 50 people who were believed to be involved in the 12 events that are under its investigation.
The investigation has been named Operation Hillman.
The Met Police said its officers would start getting in touch with the relevant people by the end of the week as they progressed their investigation “at pace”.
Those contacted are reportedly being asked to initially fill out a questionnaire.
